Bottles Entry Form

Bottles of Hope Entry Form

Traditionally, bottles used for Bottles of Hope are made over emptied/sterilized medicine bottles. If you do not have access to one of these bottles through local polymer clay guilds hosting a B.O.H. program, any glass bottle 4” in height* or smaller (*before covering).Insulin bottles are also good for this project. There is no requirement for the diameter of the bottle. Each bottle must have a hand-made lid or stopper to accompany it.

Each bottle and accompanying top/stopper must be made using Fimo Soft or Fimo Classic clays to be eligible. Mixed media use is strongly encouraged and extra consideration for using other AMACO products will be awarded (such as WireMesh,® Easy Metal Leafing®, etc.) especially in Best Use of AMACO Mixed Media. For a full listing of Amaco products please visit www.amaco.com and click on the Arts & Crafts pages.

Multiple submissions are acceptable, but each bottle submitted must be accompanied by a separate entry form. Please circle one category for each entry.

  • Best Use of FIMO
  • Best Use of AMACO Mixed Media
  • Best Children’s Theme
  • Best Expression of Hope

(All 5 finalists in each category will be eligible for Best of Show and Designers Choice)

All bottles must be received no later than June 1, 2007. In the case of multiple submissions from the same artist being selected as finalists, a jury selection of a single bottle will be made for the top 25 finalists. Please send all submissions to:
